Infant Evaluation Clinics

Infant and Toddler Evaluation Clinics are held in Bozeman and provided at no cost to children under age three in the 12 counties surrounding Bozeman, Butte, and Helena. The following information describes the process involved with this clinic.

A referral can be made by calling CDC at 1-800-914-4779. Referrals can come from anyone on behalf of a child ages 0 to 3 suspected of having delays in development (parental permission for referral required). When calling with a referral, simply say, “I’d like to make a referral to the Bozeman Infant/Toddler Evaluation Clinic”. Please have demographic information available at the time of the referral.

CDC’s administrative assistant will contact the family to schedule an appointment (clinics are held 7 months out of the year excluding summer).

CDC’s Administrative Assistant will send consent forms and request for information forms out as necessary. Copies of discharge summaries from the hospital for infants will be pursued.

Clinic attendees will see a team comprised of a nurse, cognitive specialist, speech therapist (who will also screen hearing), motor therapist, and family support specialist. The Bayley Scales of Infant Development-3rd Edition will be completed on most attendees, contingent on the reason for referral. Children are generally seen at 4-6 months of age, then 12, 18, 24 or 30 months of age.

Any infant/toddler can be referred to the Infant Evaluation Clinic if a health care provider feels he/she is in need of developmental follow along. However, please refer to the back of this page for more specific referral guidelines.
