How to Make a Referral

Referrals to any CDC program come from a variety of sources, including, but not limited to, the family or self-referrals, physicians, nurses and other medical professionals, public health professionals, other community-based disability service providers, school personnel, mental health professionals, therapists, and childcare providers.

However, families are always informed about, and need to be in agreement with, the referral prior to any further activities of intake being completed.

All CDC services are voluntary.
